Vital Records Request Service
Certified Copies
The Vital Records Request Service allows users to request certified copies of the
following vital record certificates: birth, death, marriage, civil union, divorce, or civil union dissolution.
What you should know about this Request Service:
You may order as many copies or different certificates as you want on one purchase order.
Each certified copy costs $12.00 ($10.00 statutory fee plus a $2 on-line service fee).
Most orders will ship via standard US mail within 5 to 7 business days of your request.
Expedited shipping is available at an additional flat rate of $12.50. Expedited orders will
be shipped overnight on the day your order is fulfilled. If your order can't be fulfilled,
you will receive a partial refund (you pay only the search fee and the on-line service fee
for any copy ordered but not shipped).
